　胸水，腹水が多量に貯留し，呼吸困難を来した卵巣過剰刺激症候群の症例に，胸腹水を穿刺し血液製剤の投与の代わりに胸腹水濾過濃縮再静注を行った．胸腹水穿刺により呼吸不全は改善し，胸腹水濾過濃縮再静注により循環は安定に保て，血液製剤の使用も減らせることができた．胸腹水が多量に貯留し，呼吸困難，低蛋白血症，尿量の減少が認められ管理に難渋する卵巣過剰刺激症候群に対し，胸腹水濾過濃縮再静注は呼吸循環の安定だけでなく，自家蛋白投与の点からも安全で有用な方法の一つであると思われる．

A case of a patient with severe ovarian hyperstimula-tion syndrome (OHSS) who complained of dyspnea due to ascites and pleural effusion. We removed the effusion fluid, concentrated it, and re-infused it. The symptoms and general conditions improved markedly. The patient was able to he extuhated on the 4th day. No side effects were noticed during and after. We conclude that the reinfusion system is effective in correcting the maldistribution of fluid and proteins without the use of heterogeneous biological material.
