目的

　慢性腎不全患者の血液透析における心血管系におよぼす影響については，循環血漿量や電解質，窒素代謝物の変化等，種々の因子の複雑な関与が考えられる。また腎不全による代謝性アシドーシスの是正も透析療法において重要な治療目的であるが，Mionら^1）の報告以来緩衝液として用いられているacetateは，末梢血管拡張作用や心機能抑制作用が指摘され^2），循環動態への影響も大きいものと考えられている。一方bicarbonate緩衝液は，カルシウムやマグネシウムと沈澱をおこし透析管理上煩雑であるため利用されなかったが，低血圧ショックや不均衡症候群による諸症状の発現頻度が少ないとされ^3〜6），asymptomatic dialysisとして，様々な工夫が加えられ広く普及しつつある。

　しかしacetate透析とbicarbonate透析の左心機能におよぼす影響の相違については，未だ詳細な報告は少ない。今回我々は，通常はacetate透析をしている患者にcross over方式でbicarbonate透析も施行し，それぞれの透析前後の左心機能の変化を心エコー法を用いて比較検討したので報告する。

Recently, it have been reported that the acetate dialysate have vasodialatory action and depresant action on cardiovascular system.

By using echocardiography, the hemodynamic changes of before and after hemodialysis were evaluated in two differential dialysate with cross-over method. One was usual acetate containing dialysate and another was the bicarbonate dialy-sate. Subjects were 19 patients (11 males and 8 females) who regular hemodialysis were performed, aged 22 to 66 years old. Appratus was HITACHI EUB-10. The following were evaluated; Heart rate (HR), mean blood pressure (m･BP), stroke index (SI), cardiac index (CI), diastolic descent rate (DDR), left atrial dimension (LAD), aortic excur-sion (AoEx), ejection fraction (EF) and mean velocity of circumferential fiber shortening (m\Vcf).

HR, m･BP, SI, CI, DDR, LAD, AoEx and m･Vcf changed more significantly in using the acetate dialysate than in using the bicarbonate dialysate after hemodialysis.

These results suggested that the acetate dialysate affected more remarkably than the bicarbonate dialysate on left ventricular function in hemodialy-sis.
