要旨●潰瘍性大腸炎における異形成（dysplasia）と癌の診断において，病理組織学的診断の果たす役割・期待は大きいにもかかわらず，診断に悩む病理医は多い．多彩な病理組織像を呈するdysplasiaは簡単には説明しづらく，散発性腺腫や再生異型との鑑別はしばしば難しい．dysplasiaに比較的共通してみられる所見は，“核腫大し，核小体がわかりにくい程度までヘマトキシリンに濃染する腫瘍細胞”，“腺管密度の上昇が目立たない分岐の乏しい腺管”，“p53免疫染色の異常”である．頻度は高くないが，dystrophic goblet cellsや多数の内分泌細胞といった，分化した細胞が出現することはdysplasiaに特徴的な所見である．

　The histological diagnosis of dysplasia plays a crucial role in the management of patients with inflammatory bowel disease ; however, the diagnostic criteria are still confusing, and distinguishing sporadic adenoma from reactive epithelial change can be challenging. Increased nuclear-to-cytoplasmic ratio and diffuse nuclear hyperchromasia, uncrowded and weakly branched straight glands, and p53 abnormality are the common histological features of dysplasia. Dystrophic goblet cells and neuroendocrine differentiation are characteristic to dysplasia.
