要旨　長期間経過した潰瘍性大腸炎患者は大腸癌発生の危険性が高い．また，dysplasiaは浸潤癌へ進む過程における先駆病変と考えられている．そこで，dysplasiaや早期癌の発見を目的に大腸内視鏡下の生検によるサーベイランスが行われている．しかし，現在の大腸内視鏡検査によるサーベイランスでは十分な成績は得られていない．さらにサーベイランスの成績を上げるためには隆起したDALM（dysplasia-associated lesion or mass）だけではなく，見つけにくいflat dysplasiaも見逃さないようにしなければならない．そのためには，flat dysplasiaの所見であるわずかな色調の変化，表面構造の変化，腫瘍性pit patternを注意深い観察により指摘することが必要である．

　Patients with long-term ulcerative colitis (UC) have an increased risk for colorectal cancer. Dysplasia is believed to be a precursor lesion in the pathway toward the development of invasive cancer. As such, surveillance programs using colonoscopic biopsy specimens to detect dysplasia and early cancer have been implemented. However, these surveillance colonoscopies haven't provided satisfactory results in prognosis. It is desirable to detect not only DALM but also flat dysplasia to obtain a more favorable prognosis. We should devote attention to detecting faint red mucosa, disorder of surface structure and neoplastic pit pattern which phenomena are related to flat dysplasia.
