要旨　大腸の虚血性病変の原因は，腸管壁の血流低下と腸管内圧上昇，大腸の閉塞ないし狭窄，静脈硬化症，静脈炎，細菌感染，そして薬剤などさまざまである．肉眼像，組織像が酷似している場合が多いが，それぞれの疾患に特徴的な所見もあるため，これら疾患に対する十分な病理学的知識に加え，病歴，罹患部位，便培養検査などの臨床情報の把握も鑑別診断上必要不可欠である．

　The various causes of colonic ischemia are as follows ; decreased blood flow in the colonic wall and increased intraluminal pressure, obstruction or stenosis, phlebosclerosis, phlebitis, bacterial infection, and drug-induced factors. The macroscopic and microscopic features are similar in each disease, but the special characteristics of each disease can also be found. Therefore, we should understand the pathologic features of these diseases and should have clinical information such as case history, location of the disease, and culture of the stool.
