小腸潰瘍の病理診断―非腫瘍性疾患を中心に

Pathological Diagnosis of Small Intestinal Ulcers ; Discrimination of Non-neoplastic Lesions

要旨　小腸潰瘍の病理診断について代表的な非腫瘍性疾患〔Crohn病，腸結核症，腸管Behçet病・単純性潰瘍，非特異性多発性小腸潰瘍症，NSAIDs起因性小腸病変，虚血性小腸炎─特に狭窄型虚血性小腸炎，結節性動脈周囲炎，関節リウマチ，IgA血管炎（Schönlein-Henoch紫斑病），エルシニア腸炎，腸チフス，サイトメガロウイルス感染症〕の肉眼像と組織所見を中心に概説し，鑑別の要点を述べた．小腸潰瘍の肉眼像と組織所見は疾患ごとに特徴を有している．したがって，鑑別診断を行う際には，これらの特徴を十分に把握することが重要である．特に，非腫瘍性疾患では病期による変遷や，治療による修飾を考慮する必要があるため，臨床情報を加味したうえで総合的に判断することが肝要である．

　We reviewed the histopathological findings and point of discrimination of major non-neoplastic small intestinal ulcers. Macroscopic and histological findings of non-neoplastic small intestinal ulcers have different characteristics in each disease ; therefore, it is important to understand these characteristics thoroughly for differential diagnosis. It is crucial to establish a comprehensive diagnosis correlating histopathological findings with clinical information because it is necessary to consider the changes in the lesions due to the stage of the disease and treatment, especially in case of non-specific small intestinal ulcers.
