直腸微小カルチノイドの1例と本邦報告例の文献的考察

A Case Report of Rectal Carcinoid and an Analysis of Rectal Carcinoids Reported in Japan

　直腸カルチノイドは本邦では最近まで比較的まれな疾患とされていたが，大腸二重造影や大腸内視鏡の発達とともに報告例が急増しつつある．最近われわれは術前診断が可能であった直腸微小カルチノイドの1例を経験したのでここに報告する．併せて1978年までの本邦報告例68症例を集計し，その病態と臨床像について文献的考察を加えた．

　A 54 year-old man was admitted with symptoms of nausea, abdominal pain and diarrhea. No abnormalities were found on physical examination. Radiological and endoscopical study revealed a small sessile polypoid lesion on the left lateral wall of the rectal ampulla. The tumor was covered with normal rectal mucosa, and its surface was smooth. Biopsy specimen showed argyrophil carcinoid histologically. The tumor, measuring 0.5×0.6 cm in width, was excised. The invasion of the carcinoid was limited to the submucosa. Furthermore, electron microscopic examination revealed numerous neurosecretory granules in the cytoplasm of the tumor cells characteristic of carcinoid tumors.

　69 reported cases of rectal carcinoids in Japan including the above-mentioned are reviewed.
