要旨　von Recklinghausen病に罹患した50歳，女性．大量下血で緊急入院，上部消化管は異常なく大腸内視鏡検査では回腸末端まで血液が充満していた．CTスキャンでは空腸に25mmのsolid massが同定された．手術所見ではTreitz靱帯から50cmの空腸に，固有筋層から発生した20mmの腫瘍が管外性に発育しており，ほかにも十数個の腫瘍がみられた．組織学的には多発性の平滑筋腫であった．腫瘍粘膜のびらん面から出血していた．小腸腫瘍の診断にはCTスキャンが有用であったが，von Recklinghausen病の患者には便潜血のスクリーニングを行い，必要なら小腸の精査をも行うべきである．von Recklinghausen病はneurocristopathyまたはhamartomaが本態と考えられているが，mesoderm由来の平滑筋細胞の多発性腫瘍が合併してみられたことは，von Recklinghausen病の病因は単一ではなくheterogeneityがあることが示唆された．

　A 50-year-old woman with von Recklinghausen's disease was admitted to Hakodate Chuo Hospital because of massive hematochezia. Endoscopic examinations revealed the blood-filled terminal ileum in otherwise normal gastrointestinal tract. Computed tomography revealed a mass, 20 mm in diameter, in the jejunum. Examination of surgically resected materials showed multiple leiomyomas in the jejunum, which was confirmed to be the cause of massive hematochezia. Computed tomography was useful in diagnosing the mass lesion in the small intestine. Several cases of von Recklinghausen's disease accompanied by multiple leiomyoma of the small intestine have been reported in Japan. Fecal occult blood examinations are necessary in a patient with von Recklinghausen's disease, and when indicated, examination of the small intestine is required. The pathogenesis of von Recklinghausen's disease is considered heterogenous, because multiple leiomyomas of the jejunum arise from the mesoderm.
