深達度の判定が難しかった胃底腺領域の早期癌の1例

Early Cancer in Fundic Gland Area Having Difficulty in Estimation of Invasion Depth, Report of a Case

　胃底腺領域の早期癌は診断が難しく，しかも粘膜下を浸潤するタイプが多く，診断の確定した時点では既にかなり進行しているケースが多い．われわれは胃底腺領域の早期癌1例を経験したので，胃X線検査と胃内視鏡検査を中心に若干の検討を試みた．

　We have difficulty in the estimation of invasion depth in early cancer of the fundic gland area and many cases of cancer in this area invade beneath the submucosal layer as cancer of “linitis plastica” type.

　In our case cancer is at the major curvature side in the upper part of the body.

　In roentgenography we were able to depict the folds at the posterior wall site relatively completely by double contrast figure in supine position, but depiction of folds at the anterior wall site was incomplete. We estimated the invasion depth as deeper than mucosal or submucosal layer invasion due to high degree of fibrosis in the submucosal layer.

　To depict folds at the anterior wall site more clearly we think that the double contrast figure in the prone position is necessary.

　The fiberoptic gastrocamera depicted the folds relatively well, covering the changes by fibrosis, so that we were able to estimate the invasion depth more faithfully.

　We have re-recognized the importance in the comprehension of the nature of folds themselves.
