術前，潰瘍性大腸炎，腸結核と紛らわしい所見を呈したクローン病の1例

Crohn's Disease that Presented the Indistinguishable Findings from Both Ulcerative Colitis and Tuberculous Colitis before Operation, Report of a Case

　腸の炎症性疾患の診断はX線診断，内視鏡診断の進歩によってかなり微細な所まで描写されるようになってきた．しかし実際診療に際しては，これら描写された潰瘍群をどのように診断し治療していけばいいのか，戸惑うことも決して少なくない．

　大腸の炎症性疾患，潰瘍性大腸炎，クローン病，腸結核についても，なかなか確診のつかない症例をわれわれは数例経験している．X線検査についてみると，これらの症例の中には，1つの疾患の特徴的所見だけでなく，ほかの疾患の特徴を同時に持ち合わせているもの，あるいは，経過観察中にそのパターンが変転し，他の疾患を否定できないものがある．われわれは術前クローン病を疑いながら，潰瘍性大腸炎，腸結核を完全に否定できなかったクローン病の1例を経験したので報告する．

　This report presents a case of Crohn's disease which was roentgenologically indistinguishable from both ulcerative colitis and tuberculous colitis before operation. The patient, however, showed the characteristic features of Crohn's disease on recurrence after operation. We have therefore keenly realized that definite differential diagnosis of Crohn's disease from both ulcerative colitis and tuberculosus colitis should be made based on making the double contrast follow-up study of the colon.
