要旨　患者は29歳，男性．原因不明の消化管出血に対してカプセル内視鏡を施行した．小腸にびらんを有するSMT様病変を認めた．小腸X線造影では骨盤内回腸に10mm大，類円形の隆起性病変として描出され，腹部造影CTでは強い造影効果を有する腫瘤として指摘された．GISTもしくは血管腫の術前診断で小腸部分切除を施行した．回腸末端から約180cm口側に凝血塊の付着した径10mm，弾性やや硬な白色調の隆起性病変を認めた．切除標本の病理組織では粘膜下層から粘膜面に露出し，びらんを伴う肉芽組織性腫瘤であった．深部には壁の厚い静脈壁様構造がみられ，粘膜下に位置した異常血管由来の病変で，その内部の肉芽腫様器質化と破綻が疑われた．

　A 29-year-old man with the complaint of melena underwent capsule endoscopy, which revealed a SMT-like lesion with erosion on the top in the small intestine. He was operated on and the resected specimen showed a protruding mass consisting of granulation tissue with a dilated vessel rupturing through the mucosal membrane. Angiography and histological findings showed no characteristics usually observed in vascular disease like AVM, angiodysplasia. So finally we diagnosed this lesion“Vascular malformation with a thrombus being formed”.
