Most gastric MALT lymphomas achieve complete remission due to eradication of Helicobacter pylori (H. pylori). Recently, a MALT specific chromosomal abnormality, t(11 ; 18)(q21 ; q21), has been found and, interestingly, gastric MALT lymphomas with this abnormality are definitely resistant to eradication therapy. Moreover, almost all H. pylori-negative MALT lymphoma cases are associated with this abnormality. Some gastric MALT lymphomas seem to be related to H. pylori, but not dependent on it. These findings strongly suggest that gastric MALT lymphomas are not uniform, but contain at least three different groups. This is a very important factor influencing the choice of therapy.
