はじめに

　耳瘻孔は一般的に発生の段階で生じる先天的な疾患であり，Iidaら^1）の報告によれば発生頻度は3％程度といわれている。発生部位はSkokan^2）やCongdonら^3）により分類されており，Yamamotoら^4）の報告によると耳前部の発生がほとんどである。

　Horikiriら^5）によると，耳後部に発生したものの報告は少ない。今回耳後部に発生した開口部を持たない耳瘻孔を経験したので報告する。

　Posterior auricular preauricular sinuses are rare compared to the common anterior type with a fistulous tract. We report a case of a posterior auricular mass, pathologically diagnosed as a preauricular sinus without an orifice after excision. The patient presented with swelling behind the ear, noted by an acquaintance. Fine-needle aspiration cytology and imaging were inconclusive, leading to surgical removal for definitive diagnosis. No recurrence was observed during six months of follow-up. Differential diagnoses include dermoid cyst, epidermal cyst, and cervical cyst. Due to recurrence risk, thorough preoperative evaluation and careful surgical planning are essential for optimal management.
