乱視矯正におけるKANE formulaの有用性

Usefulness of the KANE formula in astigmatism correction

要約　目的：人工知能を用いた新しい眼内レンズ度数計算式であるKANE formulaの乱視矯正における精度を検討した。

対象と方法：対象は上天草総合病院において球面度数0D狙いで白内障手術を施行した129例246眼を対象とした。使用トーリック眼内レンズ（T-IOL）はTECNIS Eyhance Toric II Opti Blue Simplicity（DIW）（ジョンソン・エンド・ジョンソン），LENTIS Comfort Toric（LS-313MF15T）（参天製薬），Avansee Preload 1P Toric（YP-T）（興和）の3種類とした。計算式にKANE formulaを用いた群（K群）と，円柱度数計算に各社のwebカリキュレーター，球面度数計算にBarrett Universal Ⅱを用いた群（W群）の術後1か月での自覚および他覚円柱度数に差があるか検討した。

結果：術後1か月の自覚円柱度数はすべてのT-IOLについてW群とK群の間に有意差はなかった。他覚円柱度数はLS313MF15TについてK群がW群より有意に小さかった。その他のT-IOLについてW群とK群の間に有意差はなかった。

結論：KANE formulaの自覚乱視矯正精度は各社webカリキュレーターと同等であった。

Abstract　Objective：This study aimed to evaluate the accuracy of the KANE formula, a novel intraocular lens power calculation method that utilizes artificial intelligence for correcting astigmatism.

Subjects and Methods：This study included 129 patients（246 eyes） who underwent cataract surgery at Kamiamakusa General Hospital with a target spherical power of 0D. Toric intraocular lenses（T-IOLs）, such as DIW（Johnson & Johnson）, LS-313MF15T（Santen）, and YP-T（Kowa）, were implanted. Postoperative outcomes were compared between the KANE formula group（K group） and the group in which cylindrical power was calculated using each manufacturer's web-based calculator and spherical power with the Barrett Universal Ⅱ formula（W group）. The primary outcomes were subjective and objective astigmatism at one month postoperatively.

Results：One month postoperatively, there was no significant difference in subjective cylinder power between the K and W groups had no significant difference for any of the T-IOL models. The K group had significantly lower objective cylinder power than in the W group for the LS-313MF15T, whereas no significant differences were observed for the other T-IOLs.

Conclusion：The accuracy of the KANE formula for subjective astigmatism correction was comparable to that of web-based calculators.
