 Melanoma necessitates a careful treatment strategy. The prognosis of melanoma patients in Japan has greatly improved since the introduction of the immune checkpoint inhibitors (ICIs), but the standard treatment of these patients remains surgery. Notably, the peripheral surgical margin applied for progressive melanomas has been 3–5 cm, but at present the appropriate margin is considered 2 cm at most. The manner in which the peripheral surgical margin is selected has thus changed. The background of melanoma cases also differs among countries and regions. In the United States, the prevalences of superficial spreading melanoma, nodular melanoma, and lentigo malignant melanoma are high, and in Japan that of acral lentiginous melanoma is high. The updated treatment strategy for melanoma is described in this article, with consideration of these different backgrounds.
