 Basal cell carcinoma (BCC) is the most common skin cancer, and achieving optimal surgical margins for BCCs is crucial for balancing curative resection while minimizing morbidity. Western guidelines recommend specific margin widths that are often based on non-pigmented BCCs, which are more prevalent in Caucasians. Pigmented BCCs, commonly observed in Japanese patients, often exhibit distinct characteristics that affect the ideal margin selection. This review analyzes the evidence for surgical margins in BCC, focusing on the differences between Japanese and Caucasian patients, especially regarding the potential for narrower margins in pigmented BCCs. We discuss diagnostic approaches and treatment strategies, and we present illustrative cases to highlight tailored margin considerations. This review serves to inform clinicians on nuanced surgical margin approaches for BCC, emphasizing individualized treatment strategies based on patient ethnicity and tumor characteristics.
