 Reconstruction of the midface with soft tissue is not sufficient to maintain the midface projection and support the forces of mastication over the long term, as it does not replace the skeletal buttresses. When the surrounding tissue is adequately vascularized, traditional free bone grafts have shown promise in the treatment of craniofacial fractures. However, non-vascularized bone grafts are unsuitable for dental implant placement because they cause unpredictable bone resorption and may require secondary onlay grafting to correct volume deficiencies. We recommend immediate vascularized bone reconstruction for midface defects over optimizing functional and aesthetic outcomes. A multitude of vascularized bone reconstructive options are available to attain these basic goals. Plastic surgeonsʼ primary concern has shifted over time from flap survival to flap refining, with the ultimate aim being the best possible functional and aesthetic results from intricate reconstructions. Reconstruction for Midface-defect is a technique that almost certainly requires a challenging reconstruction strategy and that plastic surgeons should challenge using vascularized bone for their patients.
