Nipple Reduction Surgery

はじめに

　乳頭が大きかったり，授乳を何回も繰り返していたために大きく下垂してくることがあり，それらを小さくしてほしいという患者がいる。それらを小さくする場合には単純に乳頭をV字状に切除縫縮するV字状切除（V-cut法）が簡単で有用である（図1-a）。しかし，V字状切除では術後授乳機能が失われるので，乳頭基部でドーナツ状に側壁を採取するLewis法がよく用いられる（図1-b）。また，単純にドーナツ状に採取し縫縮するRegnault法なども有用である（図1-c）。いずれの術式においても，切除し過ぎないようにすることが重要である。これらの術式とその適応について説明する。

　Some individuals with large nipples want to reduce their nipples’ volume and obtain a smaller shape.

　The simple and commonly used method for nipple reduction is the V-shape resection of the nipple, but over-resection is sometimes a problem. To avoid over-resection, we place a needle at the neck of the nipple’s 2- to 3-mm upper area from the nipple base. We cut a V-shape at the nipple-tip tissue up to the needle, and we leave the tissue under the needle alone. This protocol avoids over-resection of nipple.

　If a patient wants to be able to breastfeed after a nipple reduction surgery, we resect doughnut-shaped tissue surrounding the nipple neck, leaving the lactiferous ducts intact, similar to the Lewis method and Regnault method.
