 Alar-reduction surgery aims to create less-prominent alares while avoiding unnatural shapes or noticeable scars. It targets large nostrils, large alar size or flaring, and/or a wide alar base. An alar-reduction evaluation includes the assessment of the nostril shape, alar size (horizontal and vertical), and inter-alar distance. Based on these assessments, the surgical plan involves the excision of excess tissue while considering the patientʼs nostril shape, alar size, and scar visibility.

 Techniques include nostril excision at either the nostril sill or corner, lateral-wall wedge skin excisions to reduce the horizontal size, and alar flare and superior skin excision along the super-alar groove for vertical size reduction. An inter-alar distance approximation is also performed with the use of autologous tissue such as fascia or perichondrium.

 The process is highly individualized, ensuring optimal and natural results for each patient.
