扁平乳頭修正における風車状皮弁利用の工夫

Pinwheel Flaps for the Correction of a Flat Nipple

はじめに

　大きいと感じる乳輪を小さくする手術として，乳輪をドーナツ状に切除し縫縮する方法が行われる。その1つとして，乳頭の基部で乳輪をドーナツ状に切除する方法がある。しかしこの方法では，縫縮部が引き伸ばされて瘢痕が広がるだけでなく，乳頭が周囲に引き伸ばされて扁平化してしまうことがある。他院での術後に，このような変形の修正を目的に来院する例は多い。このような症例に対し著者らは，乳頭周囲に生じた瘢痕を風車状の皮弁として利用する方法を工夫し，ほぼ満足する結果を得たので報告する。

　After an areolar reduction with the resection of doughnut （toroidal）-shaped areolar skin at sites other than the nipple neck or areolar external area, a non-reduced areola with a scar and a flat nipple is sometimes observed. The correction of a flat nipple can be quite difficult. We have attempted the lunar flap elevation of a scar at the surrounding nipple base. The nipple neck made by this scar flap is like a toy pinwheel. We have been able to correct many flat nipples by fastening three small pieces of a surrounding areolar neck scar flap in a configuration resembling a pinwheel. Excellent results were obtained, making nipple necks and recreating a standing nipple.
