手の変形性関節症に対する診断・評価ツール

Clinical assessment instruments for hand osteoarthritis

要旨：手の変形性関節症（hand OA）はheterogeneousな疾患である。Hand OAは高い有病率を示すが，この中でも特に疼痛・手指機能障害などの症状を呈する患者（symptomatic hand OA）をpick upすることが重要となる。現在symptomatic OAの診断にAmerican College of Rheumatology classification criteria（ACR criteria）が広く用いられている。また，今日の診療，研究の場では，患者自身の主観的な機能評価が重要視されており，患者立脚型評価票（patient-rated outcome measurement；PROM）の必要性が広く認知されている。残念ながら，本邦にてhand OAに対して妥当性の担保された，疾患特異的なPROMは存在しない。Hand OA研究ではAustralian/Canadian Osteoarthritis Hand Index（AUSCAN）およびFunctional Index for Hand Osteoarthritis（FIHOA）が広く用いられている。当教室ではhand OAの “the preliminary instrument to assess physical function” とされるFIHOAを日本に導入するため，日本語版FIHOAの作成と妥当性検証研究を行っている。
