画像診断により不要な肝切除を回避し得た肝副腎遺残腫瘍の1例

A case of hepatic adrenal rest tumor in which hepatectomy was avoided by preoperative biopsy

肝副腎遺残腫瘍（hepatic adrenal rest tumor：HART）は，胎生期の副腎皮質組織が肝内に遺残し腫瘍化したものである^1）。HARTは極めてまれな腫瘍であり，また画像上肝細胞癌（hepatocellular carcinoma：HCC）との鑑別が困難であるため，HCCの術前診断で外科的肝切除が行われ病理組織学的にHARTと診断された症例が過去に多数報告されている^2）。今回我々は，C型慢性肝炎の精査中に発見されHCCとの術前診断で肝切除が予定されたものの，適切な画像診断により不要な肝切除を回避し得たHARTの1例を経験したので報告する。

We report a case of hepatic adrenal rest tumor（HART）diagnosed preoperatively, avoiding unnecessary surgery. An woman in her 80s with chronic hepatitis C presented with a 17-mm hepatic mass in segment 7. Imaging showed early enhancement, washout, and fat content, strongly suggesting hepatocellular carcinoma（HCC）. However, given the tumor’s specific location near the right adrenal gland, HART was suspected. A tumor biopsy was performed, confirming the diagnosis of HART immunohistochemically. Although HART mimics HCC radiologically, recognizing its characteristic location is crucial for accurate diagnosis and appropriate management.
