胸腺腫は胸腺上皮細胞から発生する腫瘍であり，縦隔腫瘍の20～30％を占める^1）。胸腺嚢胞は多くは先天性で，第三鰓嚢由来の胸腺咽頭管の遺残であり単房性嚢胞の形態を呈する。今回我々は，胸腺嚢胞壁に胸腺腫を合併した1例を経験した。胸腺嚢胞壁にはまれにCTやMRIなどの画像上充実成分を伴って診断に苦慮することがあるので，若干の文献的考察を加えて報告する。

The patient was a 19-year-old male. CT and MRI showed a cystic mass in the anterior mediastinum with a solid component in the cyst wall. Surgical resection was performed, and the pathological diagnosis was thymoma arising from a unilocular thymic cyst. When a thymic cyst is accompanied by a solid component, the possibility that the component is a thymoma should be considered.
