今回我々は，原発性硬化性胆管炎（primary sclerosing cholangitis：PSC）の経過観察中に胆管内乳頭状腫瘍（intraductal papillary neoplasm of the bile duct：IPNB）を生じ，肝破裂と腹膜播種をきたしたと考えられるまれな症例を経験したので報告する。

We report a rare case of intraductal papillary neoplasm of the bile duct（IPNB）causing hepatic rupture and peritoneal dissemination in a female patient in her 50s with primary sclerosing cholangitis（PSC）and ulcerative colitis. MRI showed worsening intrahepatic bile duct dilatation with a protrusion in the left lobe, suggesting impending rupture. Detecting intraductal defects and bile duct protrusion on MRI may help diagnose IPNB early in PSC patients.
