Progressive pulmonary fibrosis(PPF)is defined as meeting at least two out of three criteria―worsening symptoms, radiological progression, and physiological progression―occurring within the past year, with no alternative explanation in a patient with interstitial lung disease other than idiopathic pulmonary fibrosis. Quantitative analysis of HRCT is valuable for identifying radiological progression. In cases of fibrosis with a usual interstitial pneumonia pattern, the rate of increase in fibrotic lesions is minimal due to alveolar collapse. Therefore, attention should be given to the reduction in normal lung volume and architectural distortion of the peripheral lung structures to detect subtle fibrotic progression.
