Clinical utility of transbronchial lung cryobiopsy in the management of interstitial lung disease

びまん性肺疾患の病理診断のゴールドスタンダードは外科的肺生検であり，特発性間質性肺炎の病型分類・指定難病の申請に必要である。しかし，外科的肺生検は，侵襲性や術後急性増悪をはじめとした合併症への危惧，併存症や加齢などによる制限，検査のための手術という面での同意取得困難などから施行症例が限られ，実臨床では生検が行われず病理診断がないままに臨床画像情報をもとに診療が行われることが多い。

Transbronchial lung cryobiopsy（TBLC）is superior to conventional forceps biopsy in diagnostic yield of interstitial lung disease, and slightly inferior to surgical lung biopsy（SLB）in diagnostic yield whereas superior in safety. In this article, three cases that both TBLC and SLB was performed were presented, and the clinical utility and limitation of TBLC in diagnosis or management determination were discussed.
