骨内神経鞘腫は原発性骨腫瘍の中で0.2％未満とまれな腫瘍である。これまでに約200例報告されており，多くが下顎骨や上顎骨，仙骨，椎体骨に生じる^1）。今回我々は，非常にまれな頭蓋底から上位頸椎にかけて発生した骨内神経鞘腫の1例を経験したので，文献的考察とともに報告する。

We report a case of intraosseous schwannoma involving the skull base and upper cervical spine in a male in his 30s. MRI and CT images revealed a large lobulated mass lesion that extended both inside and outside the skull base and spinal canal. Osteolytic changes were observed in the right transverse process and lateral mass of the atlas, without narrowing of the internal penetrating artery. The mass lesion was surgically excised and diagnosed as a schwannoma. When encountering a lobulated osteolytic lesion without narrowing of the internal penetrating artery, intraosseous schwannoma should be considered in the differential diagnosis.
