Some private health insurance in Japan requires at least 50 Gy in total dose for radiotherapy(herein after referred to as “total radiation dose regulation”)as a condition for insurance payments. This regulation is sometimes problematic when it comes to hypofractionated radiotherapy, though it is unclear how many insurances are applied. We conducted a questionnaire survey of private insurance companies and revealed that 80% of all responding companies have clients to whom this regulation applies. However, we also found that up to 80% of these companies would consider insurance payments under certain conditions such as hypofractionated radiotherapy for breast cancer. This is the first survey of total radiation dose regulation in private health insurance companies.
