多房性胸腺嚢胞を伴った前縦隔セミノーマの1例

A case of anterior mediastinal seminoma with multilocular thymic cyst

多房性胸腺嚢胞は，種々の炎症後の変化としてみられる後天性の嚢胞であり，胸腺上皮の過形成に続発する反応性変化によって生じる。前縦隔腫瘍による炎症に続発することも多く，若年男性に好発する。多房性胸腺嚢胞を合併する疾患の鑑別は多岐にわたるが，まれにセミノーマと合併することもあり，若年者の多房性嚢胞性腫瘤に充実部を伴う場合はその考慮が重要である。また多房性胸腺嚢胞自体は悪性腫瘍ではないため，生検部位にも注意が必要である。我々は多房性胸腺嚢胞を合併した前縦隔セミノーマの症例を経験したため，文献的考察を加えて報告する。

Multilocular thymic cysts（MTC）are considered to be acquired cysts associated with various inflammatory processes and/or malignant tumors. We report a case of a 17-year-old male with mediastinal seminoma associated with MTC. Contrast-enhanced CT revealed an anterior mediastinal mass with 76 mm diameter, consisting of multilocular cysts and solid portion. CT-guided biopsy of the septum of the lesion revealed no malignancy. The tumor was subsequently resected. Postoperative pathology revealed a diagnosis of mediastinal seminoma with MTC. Seminoma is an important differential diagnosis of anterior mediastinal multilocular cysts with solid component in young men, and it should be noted that biopsy of the cyst wall can cause false negative results.
