子宮体癌との鑑別を要した異型ポリープ状腺筋腫の1例

A case of atypical polypoid adenomyoma initially suspected of being endometrial adenocarcinoma

異型ポリープ状腺筋腫（atypical polypoid adenomyoma：APAM）は，子宮内膜型異型腺管と平滑筋組織が密接に混ざり合ったポリープ状の腺筋腫である。良性腫瘍に分類されるが，画像上子宮体癌に類似し，病理組織検査でも子宮体癌との鑑別が困難な場合がある。40歳前後の妊娠可能な年齢に好発することから，特に挙児希望のある患者では鑑別に挙げることが重要である。今回，当初子宮体癌が疑われるも詳細な病理学的評価の後にAPAMと診断された1例を経験したので，画像所見を中心に文献的考察を加えて報告する。

Atypical polypoid adenomyoma（APAM）, a rare benign uterine tumor typically occurring in young women, has histological and radiological similarities to endometrial adenocarcinoma（EA）.

Herein, we report a case of APAM initially suspected as EA in a woman in her 20s visiting a hospital with atypical genital bleeding. A polypoid mass was found in the lower uterine corpus. MRI revealed slightly high signals on T2WI and rapid contrast enhancement on dynamic contrast-enhanced MRI. Accordingly, APAM was suspected. Following the pathological examination of transcervical resection, APAM was confirmed.

A polypoid mass in the lower uterine segment in young women could be APAM.
