異型ポリープ状腺筋腫（atypical polypoid adenomyoma：APAM）は異型のある子宮内膜腺上皮が複雑な腺管をなし，線維筋性間質を伴う，比較的まれなポリープ状の腫瘤である。APAMは約10％で類内膜癌を合併すると報告されている。子宮体癌を合併したAPAMの1例を経験したので，文献的考察を加えて報告する。

We report a case of atypical polypoid adenomyoma（APAM）with endometrial cancer. A woman in her 40 s was suspected of having APAM on histopathological examination. Pelvic MRI showed two different areas in the lesion. One showed slight hyperintensity on T2WI with strong enhancement. Another showed slight hypointensity on T2WI with mild enhancement. Postoperative histopathological examination revealed APAM with endometrial cancer. APAM is a rare uterine polypoid mass that frequently occurs in young women, and rarely occurs with endometrial cancer. When MRI demonstrates different areas inside the lesion, the possibility of APAM with endometrial cancer should also be considered.
