[99mTc]Tc-pyrophosphate（^99mTc-PYP）は，急性心筋壊死ならびに心筋障害を評価する心筋梗塞シンチグラフィおよび骨疾患を評価する骨シンチグラフィ用製剤として従来から知られている^1）2）。近年，トランスサイレチン型心アミロイドーシス（遺伝性トランスサイレチンアミロイドーシスATTRvならびに遺伝子変異のない全身性野生型ATTRwt）の診断における^99mTc-PYPの高い診断能が報告され，再び注目を集めている^3）4）。ATTR型心アミロイドーシスに対する診断能は非常に高く，さらに偽陽性を呈する症例のほとんどはAL型心アミロイドーシスであるため，これを除くと診断特異度と陽性的中率はほぼ100％と報告されている。ATTR型心アミロイドーシス症例では，心筋へのびまん性の集積を認めるとされている^5）。2020年10月26日，心アミロイドーシスの診断において，^99mTc-PYPを用いたシンチグラフィが保険診療の適応となった。

^99mTc-PYP scintigraphy has been used for diagnosing transthyretin cardiac amyloidosis. However, its quantitative analysis has not been fully established. We aimed to examine the SUVs in the heart, aorta, and lungs using SPECT/CT images, and compared them with the conventional visual assessment. SUV_mean in the heart was higher in the amyloidosis group than in the non amyloidosis group. Compared with SUV_mean in the heart, the ratio of SUV_mean in the heart and aorta（SUV_ratio（H_/_Ao））provided us a better cut-off for diagnosing cardiac amyloidosis.
