まれな経過をたどった頸椎滑膜嚢胞の1例

A rare presentation of cervical synovial cyst

滑膜嚢胞は変性などにより生じることが多く，頸椎に生じるのはまれである。典型的には慢性の経過をたどるが，まれに滑膜嚢胞が急速増大し，急性の経過をたどることもある。今回我々は，非感染性の両側椎間関節炎によりC7/Th1レベル頸椎に発生した滑膜嚢胞が脊柱管狭窄をきたし，急速に進行する脊髄症を呈したため緊急除圧を要した頸椎滑膜嚢胞の1例を経験し，報告する。

Synovial cysts are rarely depicted in the cervical spine and grow slowly in many cases. We present here an interesting case of rapid increased cervical synovial cysts. The case is a 76-year-old male, numbness and weakness on both lower legs appeared and gradually worsened for two weeks. MRI and CT showed facet joint arthritis and cystic lesions on both C7/Th1 facet joints, which caused spinal canal stenosis. Decompression surgery was performed semiurgently. In this case, there was no histological evidence of hemorrhage, infection or malignancy.
