両側錐体路に異常信号をきたした橋本脳症の1例

A case of Hashimoto's encephalopathy involving the bilateral corticospinal tracts

橋本脳症（Hashimoto's encephalopathy）は1966年，Brainら^1）によって初めて報告されたまれな自己免疫性脳症である^2）。橋本脳症は多彩な臨床徴候，画像所見を呈することから診断は容易ではないが，血清中のα-エノラーゼのN末端（NH2-terminal of α-enolase）を標的とする抗NAE抗体の検出により血清診断が可能となり，疑うことができれば早期診断が可能な疾患となった^3）。今回我々は錐体路症状と両側錐体路に沿って異常信号を呈した橋本脳症の1例を経験したので，文献的考察とともに報告する。

A 45-year-old woman presented with repeated symptomatic convulsive seizures and pyramidal tracrdysfunction. Brain MRI showed hyperintensity involving the bilateral white matter and corticospinal tracts on FLAIR images and DWI. Although the patient had been untreated, the brain lesions almost disappeared after 10 months. MR imaging of Hashimoto encephalopathy can show the improvement of the lesions during the course of the disease and affect the corticospinal tracts.
