画像経過を追えた特発性肺炎症候群の1例

A case of idiopathic pneumonia syndrome

特発性肺炎症候群（idiopathic pneumonia syndrome：IPS）は造血幹細胞移植（hematopoietic stem cell trans-plantation：HSCT）後に発症する非感染性肺障害である。IPSの病態としては各種サイトカインの関与が示唆されており，発症には治療関連毒性としての移植前化学療法や全身放射線照射の影響，移植片対宿主病（graft-versus-host disease：GVHD）などの複数の因子が関与しており，造血幹細胞移植後の合併症の中でも最も予後不良な病態と考えられている。発症から死亡に至るまでの画像経過を追えたIPSの1例を経験したので，文献的考察を加えて報告する。

Idiopathic pneumonia syndrome is a non-infectious pneumonia that develops after hematopoietic stem cell trans-plantation. Multiple factors, including the effects of pre-transplantation chemotherapy and total-body radiation as treatment-related toxicities and graft-versus-host disease, have been implicated in the development of idiopathic pneumonia syndrome, and it is considered to be one of the poorest complication after HSCT. We report a case of idiopathic pneumonia syndrome in which we were able to follow the imaging course of the patient from onset to death.
