ダプトマイシン使用後に薬剤性好酸球性肺炎と成人呼吸窮迫症候群を生じ死亡に至った1例

A fatal case of eosinophilic pneumonia induced by daptomycin progressing to acute respiratory distress syndrome in a patient with interstitial lung abnormality

ダプトマイシンはグラム陽性球菌の治療に用いられる抗菌薬であるが，しばしば薬剤性好酸球性肺炎を生じることが知られている。重篤な経過をたどる症例はほとんど報告がなく，基本的には予後良好な病態と考えられる。今回我々は，interstitial lung abnormality（ILA）を背景として，ダプトマイシン使用後に好酸球性肺炎を生じ成人呼吸窮迫症候群（acute respiratory distress syndrome：ARDS）に進展し，死亡に至った症例を経験した。ILAは死亡率の上昇，薬剤性肺障害やARDSの発症リスクとなることが報告されており，呼吸不全の進行に注意を要する。

Daptomycin, which is an antibiotic commonly used for Gram-positive cocci infections, is sometimes associated with the development of eosinophilic pneumonia. While most cases have a generally favorable prognosis, we report a fatal case of eosinophilic pneumonia induced by daptomycin progressing to acute respiratory distress syndrome（ARDS） in a patient with underlying interstitial lung abnormality（ILA）. ILA has been linked to increased mortality and a heightened risk of drug-induced pulmonary complications and ARDS. Attention should be paid to worsening of respiratory disorders in patients with ILA.
