The patient was 14-year-old boy with headache and focal neurologic symptoms. MRI on the attack demonstrated hypoperfusion in the right hemisphere on ASL. Follow-up MRI revealed the phenomenon of rebound hyperperfusion. Many studies have shown initial hypoperfusion in acute phase, followed by delayed hyperperfusion in hemiplegic migraine. ASL is accessible and sensitive to detecting abnormal perfusion. We should add ASL in pediatric patients presenting with an acute brain attack.
