正中弓状靱帯切離術前後における膵十二指腸動脈瘤の破裂リスクを評価した一例―単血流モデルを用いた数値流体力学（CFD）解析の初期経験

Assessment of pancreaticoduodenal aneurysm before and after resection of median arcuate ligament：initial experience of CFD（computational fluid dynamics）analysis

従来，内臓動脈瘤の治療適応は主に瘤のサイズにより判定されてきた。ただし，膵十二指腸動脈瘤は，他部位の内臓動脈瘤と異なりサイズが比較的小さくても破裂しやすいと報告されている。今回我々は，脳動脈瘤の評価で用いられる数値流体力学（computational fluid dynamics：CFD）解析により，正中弓状靱帯切離術前後の膵十二指腸動脈瘤の評価を試み，その破裂リスク評価における有用性について検討したので報告する。

CFD analysis is the method used to assess rupture risk of cerebral aneurysm.We tried to assess rupture risk of pancreaticoduodenal aneurysm before and after resection of median arcuate ligament by CFD analysis.

Wall shear stress（WSS），one of the most important parameters of CFD analysis，is the frictional force against vessel wall.Many research show that low WSS increases rupture risk of cerebral aneurysm.In our case，WSS of pancreaticoduodenal aneurysm rised within normal range after the resection，which suggest the reduction of risk of aneurysm rupture.
