胎盤部トロホブラスト腫瘍の1例

A case of the placental site trophoblastic tumor

妊娠性胎盤性疾患（gestational trophoblastic disease：GTD）とは栄養膜細胞（子宮への胚の付着・胎盤形成に関与する細胞）を発生母地とする疾患の総称であり，胞状奇胎（完全・部分），侵入奇胎，絨毛癌，placental site trophoblastic tumor（PSTT），epithelioid trophoblastic tumor（ETT）といった様々な生物学的態度を有する疾患を含む。それぞれの疾患は臨床症状・好発年齢・β-hCG値・好発部位・先行妊娠・初発時転移・治療が異なる。存在診断において画像診断は大きな役割を果たすものの各絨毛性疾患間の決定的な画像所見の鑑別点は知られていない。しかし「侵入奇胎と絨毛癌」と「PSTTとETT」はそれぞれ「化学療法」と「子宮摘出」のように治療方針が異なるので，放射線科医としてそれらの特徴を知っておくことは重要である。

We report a case of the placental site trophoblastic tumor（PSTT）. A 30 year old woman, gestation 2, production 2, was referred to our hospital with amenorrhea. Serum β-hcg level was elevated（190.6 mIU/ml）, and US shows a low-echoic tumor in the uterus. On MRI, tumor shows high signal intensity with cystic change. Tumor also shows heterogeneous enhancement in contrast-enhanced FS T1WI. Total hysterectomy had been done and histopathological diagnosis was confirmed as placental site trophoblastic tumor.
