正常膵類似形態を呈したまれな腸間膜内異所性膵の1例

A rare case of heterotopic pancreas in the jejunal mesentery, morphologically similar to the orthotopic pancreas

異所性膵は解剖および血管支配において正常膵と異なる膵組織と定義される。異所性膵は主に上部消化管に好発する粘膜下腫瘍の一つである^1）。画像検査ではほかの粘膜下腫瘍と類似した所見を呈することがあり，鑑別はしばしば困難であるとされる^2）。今回我々は，造影CT（早期相，後期相）にて空腸の腸間膜に正常膵と類似形態を呈する腫瘤を認め，摘出後の病理所見で腸間膜内の異所性膵と診断された症例を経験した。腸間膜内の異所性膵はまれであり，特徴的な画像所見が術前診断に有用であったため，若干の文献的考察を加えて報告する。

A man in his 60s had an incidental submucosal mass in the jejunum. Contrast-enhanced computed tomography revealed a mass（4.2 cm in diameter）located in the jejunal mesentery. The mass was attached to the adjacent jejunal wall and tapered toward the mesenteric aspect on its other side. Additionally, it showed early enhancement and a duct-like structure was observed within the mass, which was similar to the orthotopic pancreas. Based on radiological findings, we suspected heterotopic pancreas. Because of the possibility of malignancy, the mass was resected by using laparoscopic approach. Histopathological examination confirmed heterotopic pancreas in the jejunal mesentery.
