特徴的な形態を呈した異所性膵の1例

Ectopic Pancreas with Distinctive Morphological Features, Report of a Case

要旨●患者は50歳代，女性．EGDにて胃体中部小彎やや後壁寄りに長径40mm強の頂部が陥凹した粘膜下腫瘍様の隆起性病変を認めた．陥凹部はひび割れ状・溝状・円形の開口部様所見を有しており，非典型的な形態を呈した異所性膵と考えた．診断的治療目的にESDにて切除し，病理組織学的に評価したところ，幼若な分化を呈した異所性膵であることが判明した．

　The patient was a woman in her 50s. During an upper gastrointestinal endoscopy, a submucosal tumor larger than 40mm in diameter was identified on the lesser curvature of the middle stomach, slightly extending toward the posterior wall. The tumor had a depressed apex characterized by cracks, grooves, and circular openings, indicating an atypical form of ectopic pancreas. The lesion was resected via endoscopic submucosal dissection for diagnostic and therapeutic purposes. Histopathological evaluation confirmed that it was an ectopic pancreas with immature differentiation.
