異所性膵は解剖学的にも血行学的にも膵臓とは連続性をもたず，他臓器中に存在する膵組織である。多くが無症状であるため，偶発的に見つかることが多い。異所性膵そのものによる通過障害や，潰瘍形成による出血，膵炎の合併といった合併症をきたすことはよく知られているが，悪性腫瘍をきたす頻度は低い^1）2）。そのため，術前の診断に難渋したとしている報告が多い。今回我々は，術前診断が非常に困難であった異所性膵発生と推察される胃の膵腺房細胞癌の症例を経験したので，文献的考察を加えて報告する。

We report the case of a male in his 60s patient with acinar cell carcinoma arising from ectopic pancreas in the stomach. Contrast–enhanced CT showed a submucosal mass with a depression and early enhancement at the gastric body, and FDG–PET/CT revealed slight FDG accumulation. Furthermore, the serum lipase was elevated, and upper endoscopy revealed the submucosal extension of the tumor. Following chemotherapy, the tumor size was reduced, and a total gastrectomy was performed. Pathological examination showed that the tumor was positive for BCL10 and located in the submucosa, resulting in the diagnosis of acinar cell carcinoma arising from ectopic pancreas.
