左同名半盲で発症した中枢神経ゴム腫の1例

A case of central nervous system（CNS）syphilitic gumma presented with left homonymous hemianopia

梅毒はペニシリンによる治療法が確立されて以降，発生数は激減した。しかしながら，その後も各国において幾度かの再流行がみられ，我が国においても2010年代より発生数は再び増加傾向に転じており，過去の病気とは言いがたい^1）。このように梅毒患者が増加している中で，今回我々は鑑別に苦慮した中枢神経ゴム腫の1例を経験したので，若干の文献的考察を交えて報告する。

We report a case of a CNS syphilitic gumma patient who presented with visual field impairment. MRI demonstrated multiple enhancing nodules distributed in the right aspect of the pons, right thalamus, and posterior limb of the right internal capsule, as well as the left trigonal white matter and genu of the corpus callosum. The diagnosis of CNS gumma was confirmed with the positive result of RPR. The diagnosis of CNS syphilitic gumma can be challenging from neuroimaging due to limited evidence of imaging finding’s accuracy, but it is important to include it in differentials for further diagnostic procedures and treatments.
