脳転移を示した副腎CD3陽性びまん性大細胞型B細胞性リンパ腫の1例

A case of brain metastases with adrenal CD5 positive diffuse large B cell lymphoma（DLBCL）

副腎に原発したCD5陽性びまん性大細胞型リンパ腫（DLBCL）が，中枢神経系転移を伴った症例は非常にまれである^1）。今回，我々はFDG-PET/CTなど画像診断および病理検査を施行したので若干の文献的考察を加え報告する。

We report an age 60 woman with rare CD5 positive adrenal DLBCL with brain metastases. Brain metastases have enhancement on MRI and FDG accumulation on PET/CT. PET/CT revealed a left adrenal gland accumulation, too, which is diagnosed CD5 positive DLBCL by CT-guides needle biopsy. Adrenal lymphoma appeared T2 shine-through phenomenon on MRI which may be due to ischemic change of venula occlusion by lymphoid cells. PET/CT could detect both lesions and decided adequate therapy for patients with CD5 positive DLBCL.
