unroofed coronary sinus（UCS）は心房中隔欠損症（atrial septal defect：ASD）のうち1％以下と非常にまれな疾患で報告数は少ない。先天性疾患ではあるが，生下時に指摘することが困難かつ身体所見も非特異的な疾患である。今回我々は，心臓CTで診断されたUCSの1例を経験したので報告する。

A 87 year-old woman visited our hospital complaining lower extremity edema and dyspnea. Transthoracic echocardiogram showed enlargement of left atrium and a 4.6cm thrombus in left atrium. Cardiac CT revealed CS dilation and absence of coronary sinus roof. Then, we had a diagnosis of unroofed coronary sinus type 2. In this case, cardiac CT was useful for diagnosis.
