小児心臓CTの被ばく低減の考え方と低線量撮影の実際

小児心臓CTというと多くの放射線科医は無縁の世界だと考えていることが多いが，大学病院はもちろん，一般病院でも，小児科がある病院では先天性心疾患の小児を撮影する機会は突然訪れる。先天性心疾患の有病率は1％，複雑先天性心疾患で0.1％程度と高い。また，大学病院や専門病院で手術を終えた子どもたちが，医療連携や転居により地域の病院に受診する機会も増えているからである。CTの寝台に寝ているほんの小さな子ども，速い心拍数，わかりにくい血行動態。診断参考レベルにも小児心臓CTの線量は書いていない。年に1回あるかないかの検査を前に，失敗してはいけないと，成人並みの線量で何心拍も何相も撮影してしまう，というのはよく聞く話である。

Since pediatric patients with complex heart disease undergoes a sequence of invasive angiography and interventions with radiation exposure, exposure reduction in CT is a requisite. Being the modality of highest exposure in the past, modern pediatric cardiac CT has become the modality of the lowest exposure, as low as the exposure of plain chest film. This paper explains the basic strategies to reduce the radiation exposure of pediatric cardiac CT, and also introduces the summary of published evidences and actual ultra-low dose CT images in the authors’ institution.
