小児に限らず，医療被ばく低減の2大原則は「最適化」と「正当化」である。最適化とはその患者の体のサイズに見合った線量で検査をすることである。体の小さい子どもや小柄な成人では体の大きなサイズの成人ほどの線量は不要である。また正当化とは本当に必要な検査，施行する意味のある検査を行い，不必要な検査は行わないことである。

The two main principles of reducing medical radiation exposure, not just for children, are “optimization” and “justification.” Optimization means performing examinations at a dose appropriate to the patientʼs body size. Small children do not need the same dose as larger adults. Justification means performing truly necessary examinations and those that make sense, while not performing those that are not necessary. This article explains the latest information on five key points to keep in mind when optimizing radiation exposure doses for children：the ALARA principle, diagnostic reference levels（DRLs）, Japanʼs DRLs 2025, diagnostic imaging guidelines, and the abolition of gonadal protection.
