Methotrexate-associated lymphoproliferative disorders（MTX-LPDs）：their clinical, radiological, and pathological manifestations

メトトレキサート（methotrexate：MTX）は，葉酸代謝拮抗作用をもつ抗腫瘍薬，免疫抑制薬であり，現在では関節リウマチ（rheumatoid arthritis：RA）治療における中心的薬剤として広く用いられている^1-3）。2011年には本邦においてMTX上限16mg/週が承認され，MTX使用量も増加してきている。さらに近年では，RAの予後不良因子をもつ症例に対して，種々の生物学的製剤やJanus kinase（JAK）阻害薬などの分子標的治療薬と併用して使用されることも推奨されており^1-3），RA治療に欠かせない薬剤である。

Methotrexate（MTX）, which is one of the most commonly used disease-modifying anti-rheumatic drugs（DMARD）, is a key treatment for rheumatoid arthritis（RA）. RA patients are at higher risk of developing lymphoma. Furthermore, RA patients who are treated with MTX can also develop lymphoproliferative disorders（LPDs）. These disorders are referred to as MTX-LPDs, which are categorized as “other iatrogenic immunodeficiency-associated lymphoproliferative disorders” in the World Health Organization 2008 classification. Herein, we present the clinical, radiological, and pathological features of MTX-LPDs.
