子宮腺筋腫は子宮内膜組織と平滑筋よりなる良性腫瘍であり，画像上多彩な所見を示すことが知られており，しばしば診断が困難なことがある。T1強調像で高信号の出血性空洞病変を認め，付随する腺筋症を認めた際，子宮腺筋腫を鑑別に挙げる必要があるといわれている^1）。今回我々は有茎性漿膜下子宮腺筋腫の1例を経験したので，従来の知見を踏まえたうえで若干の文献的考察を加えて報告する。

Subserosal and pedunculated adenomyoma is an unusual pattern of adenomyoma. We present a case of a 45-year-old woman that can represent diagnostic challenge.

CT and MR images showed well-defined mass which contains multilocular cystic part and solid part. On MRI, the cystic part displayed hyperintensity on T1-weighted images and solid part displayed hypoinsensity on T1- and T2-weighted images and showed homogeneous contrast enhancement.

Histologically, the inner cystic cavity was lined by endometrioid tissue surrounded by the smooth muscle.
