子宮腺筋腫は2020年WHO分類で上皮性・間葉性混合腫瘍に分類される腫瘍で，子宮内膜型腺管と平滑筋からなる良性腫瘍と定義される。本例のように，高齢女性に発生し，出血を伴わない嚢胞成分主体で子宮腔内から筋層・漿膜下への広範な進展を示す特異な発育形態を呈した腺筋腫は検索した限り過去に報告がなく，極めてまれな症例と考え提示する。

We report a rare case of uterine adenomyoma in a woman in her 60s. MRI revealed a cystic tumor primarily composed of cystic components, extending from the uterine cavity to the myometrium and serosa. The cystic contents showed low signal intensity on T1-weighted imaging. The septa of the tumor demonstrated strong enhancement during the early phase of contrast. This atypical form of adenomyoma, with extensive extension to the myometrium and serosa in an elderly woman, is extremely rare.
